Yes, orgasm is the same as coming. Go for a quickie, keep in mind what your goal is this time, concentrate on this instead of getting "caught up". That's the very way you could hoodoo yourself! If you keep your head so to speak, you'll have attained your goal.
As far as otc stuff, dunno that there is anything specifically to get you to start. The gyno can give you something to jumpstart, but if they're satisfied that everything is as it should be, they probably won't. If you have polycystic ovary syndrome and that is the cause of the irregular periods, exercise can actually help regulate your periods. However, if this is just your normal pattern, you would have to wait til you have one or use your sexual fluids, as opposed to blood. I don't know that it would be as strong, but I guess it would depend on your need for strength vs. speed. Post-menopausal or women who have had a hysterectomy would have to use their sexual fluids. But, given the choice, I personally would wait for a period.
